* include/Makefile.am (${pch1a_output}): Use gnu++0x flags instead
of debug.
* include/Makefile.in: Regenerate.
* include/precompiled/stdc++.h: Add.

* doc/xml/api.xml: Add gcc-4.4 link.
* doc/html/api.html: Regenerate.
