(mf_xform_derefs_1) [RESULT_DECL, STRING_CST]: Handle as innermost
object.  [ptr_type]: Remove unused variable.
Remember nearest addressable array-element or record-component
when checking bitfield components.  Tolerate empty BIND_EXPR.
